Biometric Screening

 All Employees enrolled in our health insurance plan as well as their spouse, if he/she is also covered by the plan, must complete a free biometric screening by January 31, 2021. As a reminder, the Biometric Screening must be completed every year. To schedule your screening, click the “Book Now” button below.

NOTE: Individuals who wish to know their prior year results may obtain those by making an appointment at the IU Health & Wellness Center.

The biometric screening is designed to provide individuals with confidential feedback regarding their overall health as compared to established benchmarks within the medical community (cholesterol, blood pressure, diabetes screening, body mass index, etc.). Both the Employee and the Spouse (if covered by the plan) must complete the screening by January 31, 2021 to avoid an increase of $825 to the Employee’s health insurance premium ($75 per month, February 1, 2021 through December 31, 2021).

Additional Information

If a biometric screening has been completed by a physician after June 1, 2020, or if an individual is pregnant or one-year postpartum, you may satisfy the screening requirement by having your physician complete and fax this form to the IU Health and Wellness Center (317-968-1039).

Note: The patient will receive an email confirming the receipt of this form within two weeks of submission. Should the patient not receive a confirmation, it is the patient’s responsibility to contact the clinic at (317) 205-3332 ext. 22222, and then follow up with their physician as necessary.
