Important Information

All Employees enrolled in our health insurance plan as well as their spouse, if he/she is also covered by the plan, must complete a free biometric screening by January 31, 2019. As a reminder, the Biometric Screening must be completed every year. To schedule your screening, click the "Book Now" button below:

NOTE: Individuals who wish to know their prior year results may obtain those by making an appointment at the IU Health & Wellness Center.

The biometric screening is designed to provide individuals with confidential feedback regarding their overall health as compared to established benchmarks within the medical community (cholesterol, blood pressure, diabetes screening, body mass index, etc.). Both the Employee and the Spouse (if covered by the plan) must complete the screening by January 31, 2019 to avoid an increase of $825 to the Employee’s health insurance premium ($75 per month, February 1, 2019 through December 31, 2019).

If a biometric screening has been completed by a physician after June 1, 2018, or if an individual is pregnant or one-year postpartum, you may satisfy the screening requirement by having your physician complete and fax this form to the IU Health and Wellness Center (317-968-1039).

Note: The patient will receive an email confirming the receipt of this form within two weeks of submission. Should the patient not receive a confirmation, it is the patient’s responsibility to contact the clinic at (317) 205-3332 ext. 22222, and then follow up with their physician as necessary.

How do I schedule my biometric screening?

Scheduling instructions

  1. Click on the word Register next to the desired event in the event list.
  2. Be sure to review the event details and event description at the top of the page.
  3. Select a time from the Desired Time drop-down menu on the right.
  4. Enter your email address, verify your email address, and enter first name, last name and optional
    phone number.
  5. Click Register.
  6. An event confirmation message will appear that you can print for your records. You will also receive a confirmation email if you have provided a valid email address.

Rescheduling instructions

  • Option #1: Click on the link contained in the confirmation or reminder email. The link will take you to a sign-in page where you can create a password and sign in to see your appointments. Use the cancel or reschedule option next to your appointment.
  • Option #2: If you can’t find the confirmation or reminder emails, you may cancel/reschedule by returning to the website and clicking on the “Sign In” option in the upper right corner of the screen. This will take you through the steps listed above.

Are there any incentives for participating in the biometric screening?

You can avoid an $825 increase in your health insurance premium ($75 per month for 11 months) if you complete a biometric screening. If your spouse is on your health plan, he or she must complete the biometric screening, or your premium will increase.

When will I get my results and what happens next?

The results will be shared with you before you leave the screening. If you have additional questions or need to follow up with the health coach or physician, contact the Health and Wellness Center at Washington Township.

Can I return to work after the screening?

Yes, it is safe to return to work after your biometric screening.

How do I prepare for the biometric screening?

For the most accurate lab results, please do not eat anything for at least 12 hours before your screening appointment. Do not drink anything but water or black coffee 12 hours before your screening. You may take medications as prescribed.

How long will the biometric screening take?

Please allow 15 to 20 minutes to complete the screening.

What does the biometric screening include?

A simple finger stick will help you learn important details about your:

  • Blood pressure
  • Height and weight
  • Body mass index (BMI)
  • Waist circumference
  • Cholesterol (total, HDL, LDL and triglycerides)
  • Fasting blood glucose
  • Hemoglobin A1C

When and where will the biometric screenings take place?

Biometric screenings will take place at the Health and Wellness Center at Washington Township from Oct. 27 through Jan. 26. The center is located at Northview Middle School (Door 19A South), 8401 Westfield Blvd.

Who should participate in a biometric screening at the Health and Wellness Center at Washington Township?

You can avoid an $825 increase in your health insurance premium ($75 per month for 11 months) if you complete a biometric screening. If your spouse is on your health plan, he or she must complete the biometric screening, or your premium will increase.

Please note: If you completed a biometric screening through your physician or the Health and Wellness Center after June 1, 2018, or if you are pregnant or one-year postpartum, you do not have to complete the biometric screening. However, to avoid the premium increase, you will need to have your physician complete and fax the Physician Option Form (located at the bottom of this page) to the Health and Wellness Center (fax number is on the form).

Are the results of my biometric screening confidential?

Indiana University Health is a HIPAA-compliant organization and takes confidentiality very seriously. At no time will your individual biometric screening results be provided to your employer.

Why should I participate in a biometric screening?

The information you gain from routine biometric screenings can improve your health. “Knowing your numbers” enables you to be proactive in addressing any health issues so you can stay healthy and live better. You will learn more about your general health and any possible health risks. During the screening, you also will have an opportunity to find out about the health services available to you and ask questions or address health concerns with a medical professional.
