Care coordinators may provide psycho-educational groups, teacher/parent consultation, behavioral interventions in the home and at school, activities of daily living skills in home, school, and in the community, linkage to community and school resources for the student and family, and assistance in accessing necessary supports such as probation or the medical doctor.
