Biometric Screening Options
The biometric screening is designed to provide individuals with confidential feedback regarding their overall health as compared to established benchmarks within the medical community (cholesterol, blood pressure, diabetes screening, body mass index, etc.). Both the Employee and the Spouse (if covered by the plan) must complete the screening between January 1 - December 31 of the prior plan year to avoid an increase of $825 to the Employee’s health insurance premium ($75 per month)

Additional Information
If a biometric screening has been completed by a physician or if an individual is pregnant or one-year postpartum, you may satisfy the screening requirement by having your physician complete the Physicians Option Form.
